PARENTS AND PLAYERS
REGISTER FOR USA HOCKEY
The Cleveland Barons and USA Hockey require ALL players, coaches, managers and volunteers to register with USA Hockey.
COMPLETE SAFESPORT TRAINING
ALL coaches, managers and volunteers are required to complete USA Hockey SafeSport training or the refresher course. Any player who is a 2009 birth year or earlier is also required to take the training or refresher course.
CONSENT TO TREAT
The Cleveland Barons are REQUIRED to have a Consent to Treat form on file for EVERY player.
Please complete this form and give to your coach or team manager.
USA HOCKEY WAIVER AND LIABILITY RELEASE
The Cleveland Barons are REQUIRED to have the USA Hockey waiver and liability release form on file for EVERY player.
This should be completed during the registration process.
